Penicilliosis marneffei in HIV negative children: three case reports

: This study aimed to report the clinical characteristics of penicilliosis marneffei (PSM) in three children negative HIV. Three were diagnosed with PSM Department Emergency Medicine, Hunan Children’s Hospital between February 2016 July 2020. The characteristics, laboratory findings, and concomitant diseases recorded, related literatures reviewed. treatment reported according our experience literature review. initial symptom was right lower limb mass 1 child (first) who developed fever cough about month later then misdiagnosed tuberculosis. other (second) had a fever, reductions red blood cells, white cells platelets, hepatosplenomegaly lymphadenectasis. third jaundice, multiple organ dysfunction syndrome (MODS), first (Case 1) STAT1 gene mutation on genetic examination, second 2) history onychomycosis oral ulcer, 3) STAT3 Hyperimmunoglobulin E syndromes (HIES). confirmed all cases by culture bone marrow. All through medulloculture. Case 2 also lymph node biopsy. 3 sputum bronchoalveolar lavage fluid (BALF). intravenously administered voriconazole amphotericin B liposomes, orally itraconazole for maintenance therapy, which discontinued year later. thereafter total 7 months. Recurrence not observed. given days (discontinued due liver dysfunction), intravenous 4 days. patient gave up therapy finally. In conclusion, HIV can develop PSM, may be STAT1/STAT3 mutation. For having no response antibiotic or antiviral bacterial/fungal biopsy should performed as soon possible confirm diagnosis, physicians actively identify underlying patients, is beneficial early improvement prognosis
